Never met a Cosima IRL before (and I know a lot of elderly Italian-Americans). I did however obsessively watch Orphan Black back when that was on TV, and that show had a very cool Cosima character. They pronounced it ko-SEE-muh. Other fans of the show noted that they associated the name with British aristocracy, which Wikipedia would seem to confirm.
I think it'd be a lovely choice in a Hispanophone context, and it'd sound nice to Anglophone ears as well. Certainly a less awkward name than Cosmo.
